If you are a current member send in this form with your check at the beginning of the calendar year in order to have your membership renewed.

Community Bulletin Board Request Form

Download this form to use as a template for your Community Bulletin Board Submissions. WinCAM will promote your non-profit events and announcements free of charge.

DVD Order Form

Download this form, fill it out and mail it in, along with checks payable to wincam to recieve copies of programs aired on WinCAM channels. Please Remember to include $5 for shipping if youd like your DVDs mailed to you. You may also call or email us to order DVDs that you plan to pick up.

Cablecast Request Form

Downlaod this form and fill it out to accompany programming you would like to see aired on WinCAM. Cablecast Request Forms are required for every WinCAM program, please remember that resident sponsors may also be required for import programming.

Coverage Request Form

Download and fill out this form to request coverage of a community event. Because WinCAM depends on the effors of voulenteers, we cannot guarantee coverage of all events submitted. Please submit requests no later than two weeks prior to the event.

Download and fill out this form for everyone who is going to appear on your program.
